Before we meet in person, let’s decide how I can help you. If I feel you will benefit more from someone else’s services, I will happily direct you towards another provider.
If we decide to work together, I will help you schedule your initial evaluation.
Initial Evaluation
The first time we officially meet in the comfort of your own home, we discuss your needs and work together to set your personal goals. I take you through a comprehensive set of tests. Then we decide on a plan to achieve those goals.
Treatments
Personalized treatments are designed to help you reach your goals. This can include a combination of education, manual therapy, exercise, and more.

Also known as “fee-for-service,” Bayley Physical Therapy PLLC is a cash-based business, meaning I am not under contract with any insurance companies. I will not directly bill your insurance for services.
“Cash-based” doesn’t mean I only accept cash. You can pay with cash, credit/debit card, HSA, FSA, or personal check.

Cash-based services allow us the freedom to work together as a team to determine the best course of action for you - regardless of what your insurance company thinks you need or will cover.
However, I CAN work with your out-of-network insurance benefits. Please contact me if you have out-of-network insurance benefits and have questions about how to use them. To find out if you have out-of-network benefits, please use this Benefits Verification Guide to speak with your insurance company.
If you want to use your out-of-network provider benefits, I will provide you what is known as a “superbill” that will invoice the services rendered. You can then submit this superbill to your insurance company and await reimbursement.

Freedom:
If your goals are met in only two visits, I won’t pressure you to schedule any more. If you only want to see me once a month for maintenance, we don’t need to ask your insurance company’s permission.
Because I don’t take insurance, we don’t have the confines or productivity standards that traditional outpatient physical therapy has. You’re not sitting there doing exercises incorrectly while your PT directs someone else through exercises. I’m not getting pulled away to take a phone call or look at the schedule for another patient.
Price transparency:
You know exactly what the prices are for services without a surprise bill.
Some people are shocked by this, but many people actually spend LESS on their physical therapy when seeing an out-of-network provider because of the quality of the individualized care.

Some Medicare Advantage Plans or Medicare Replacement Plans have out-of-network provider benefits, just like commercial insurance. Use the Benefits Verification Guide to help you find out.
I do not have a relationship with Medicare. Unfortunately, Medicare rules prevent Medicare beneficiaries from paying out of pocket for covered physical therapy services. There are very specific instances when a non-covered service CAN be paid for out of pocket. Please contact me so I can help you determine how you can best get care.

No, you do not need a referral if you will not use insurance benefits or seek reimbursement via a superbill.
You do need a referral if your insurance requires one and you plan on seeking reimbursement from your insurance company. You may need a referral from your primary care provider, or from a specialist. Please use the Benefits Verification Guide for more help.

Each physical therapy session for an individual costs $225. I may offer you a package of visits at a discount.

I allot one hour to spend one-on-one with you in person.

First, you will need to have created a profile through my patient portal, linked here.
I will then send you specific intake information through the patient portal. This includes giving me details about your health history, current problems, and demographic information.
If you are not tech-savvy, I can assist you with filling out the forms on my computer when I meet with you in person. Please know this will cut into the time for our evaluation process.
Please ensure you have enough space in your home for me to set up a treatment table, along with some surrounding space to move around comfortably. Wear clothes that are loose-fitting and comfortable, understanding I may ask you to move into a variety of different positions.

Yes, I’m happy to meet your family! I love dogs and cats! I will just need access to a sink and soap in case of allergies.
I do ask that you minimize distractions. Please keep in mind that I will have a designated period of time that I can spend with you, and unfortunately I cannot stay longer if we are interrupted.

Please ensure this is okay with your boss or the gym manager BEFORE we plan a session there. If there is an entrance fee for use of the gym, it is expected that you will cover that expense.

I require 24 hours notice to cancel an appointment. If less than 24 hours notice is given, there is a $150 “Late Cancellation Fee.”
I understand that emergencies or illnesses happen. I do reserve the right to waive fees.
